Fiscal Year 2014 CMS Inpatient Rehabilitation Facilities (IRF) Prospective Payment System (PPS) Proposed Rule Changes



On May 8th, the Centers for Medicare and Medicaid Services (CMS) released the IRF PPS proposed rule changes for Fiscal Year 2014.  Among provisions in this year’s proposed rule that are of interest to infection prevention are:
  • Reporting of CAUTI through NHSN (Pages 103 in proposed rule FR 26909, online p. 31):  This measure was first adopted for FY 2012 before it was endorsed by the National Quality Forum (NQF) for IRF settings, and updated last year once the measure was NQF-endorsed.  The reporting requirement for the IRF Quality Reporting Program (QRP) remains unchanged this year, but the measure has been renamed “National Healthcare Safety Network (NHSN) Catheter-Associated Urinary Tract Infection (CAUTI) outcome measure (NQR #0138).
  •  New quality reporting measures:
    • Influenza Vaccination Coverage among Healthcare Personnel (NQF #0431) (Page 107 FR 26911, online p. 33) – Reporting period would be from October 1 – March 31 to align with this measure in other programs.  The first reporting period of this new measure would be October 1, 2014 to March 31, 2015 for FY 2016 Medicare payment determination.
    • Percent of Residents or Patients who were Assessed and Appropriately Given the Seasonal Influenza Vaccine (Short Stay) (NQF #0680) (Page 118FR 26914, online p. 36) – This measure was recommended by the NQF-directed Measure Applications Partnership (MAP) for IRF settings.  CMS is proposing to include it for FY 2017 payment determination
    • All-cause Unplanned Readmission Measure for 30 Days Post Discharge from IRFs (P. 111 FR 26912, online p. 34) – This measure estimates the risk-standardized rate of readmissions to a short-stay acute care hospital or LTACH within 30 days of IRF discharge.  This is a claims-based measure not requiring reporting of new data by IRFs, and therefore will not be used to determine IRF compliance with the IRF QRP.  CMS is proposing to begin reporting feedback to IRFs on performance of this measure in calendar year 2016.
  • Reporting timelines (P. 135 FR 26919, online p. 41) – CMS is proposing to align the IRF QRP submissions deadlines to NHSN with the submission deadlines in the Hospital Inpatient Quality Reporting (IQR) and the Long-Term Care Quality Reporting (LTCQR) programs.
